You throw a smoke bomb on the ground within range that blocks line of sight in a 5ft radius (15x15 area total) of where it was thrown. Targets inside or within 5ft of the smoke can only see up to 5ft into or out of the smoke.

If you’re inside of it at the point it's thrown, you may immediately gain the Hiding state from anyone you have cover or concealment from.

The smoke lasts until the start of your next turn. When it ends, if you are inside your smoke, you may immediately move up to your speed and you are considered hidden up until you finish that movement. If you end your movement in cover, you keep being Hidden from people you have cover from.
